Thank you for your attending one of our CE/CME programs. In order to obtain your Statement of Credit for the credits earned, you must complete the program evaluation. To access the online evaluation please follow the instructions provided below:

1. Enter your login information and the course code number (Code number provided to those in attendance) and submit.
2. Upon validation of the information entered you will be delivered directly to the online program evaluation form. Complete and submit the evaluation.
3. Upon submitting the evaluation form you will be delivered to your User Records section of this website. A Certificate link will be provided directly across from the program title. To view and/or print a Statement of Credit click the link. You may choose to print your Statement immediately or you may return to the User Records section of the CME-University.com website at any future date to view and print previously completed statements.
