INSTRUCTIONS FOR CLAIMING CME/CE CREDITS:
ONLINE PRE-REGISTERED ATTENDEES:
If you registered online prior to attending, enter the email address that you registered with and the course code (provided to all attendees) in the fields provided from within the Claim Credit page, once entered click the continue button.
To access the claim credit fields click the Claim Credit button above.
Once validated you will be prompted to enter the Password associated with your CME-U user account (provided with pre registration). Once validated, complete the CME/CE online post test followed by an online evaluation form. Once you have completed both, you will be able to view and print your Statement of Credit, which will be housed within your cme-university.org personal User Record account and available anytime in the future once logged in to cme-university.org.
NON ONLINE PRE-REGISTERED ATTENDEES:
If you attended without pre registering online enter your email address and the course code (provided to all attendees) in the fields provided from within the Claim Credit page, once entered click the continue button. To access the claim credit fields click the Claim Credit button above. You will be delivered to the Walk In Registration page and prompted to complete the program registration form.
Once you have submitted the online form, you will be prompted to complete a CME/CE post test and an evaluation form. Once you have completed both, you will be able to view and print your Statement of Credit, which will be housed within your cme-university.org personal User Record account and available anytime in the future once logged in to cme-university.org.
Purpose:
Effective management of MDS requires understanding of underlying disease-specific abnormalities as well as patient-related features. These issues as well as current standard and recent novel therapeutic advances for MDS patients will be discussed.
Accreditation:
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the Foundation for Care Management and the MDS Foundation. The Foundation for Care Management (FCM) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for Physicians.
FCM designates this educational activity for a maximum of 3.5 AMA PRA Category 1 credits™. Physicians should only claim credit commensurate with the extent of the participation in the activity.
The Foundation for Care Management is an approved provider of continuing nursing education by the Washington State Nurses Association Continuing Education Approval & Recognition Program (CEARP), an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.
